What should I bring to my initial appointment?

• Patient Registration Form

You will be emailed a link from reception to your New Patient Registration Form, and we ask that you fill this in prior to your appointment.

• Referral letter

A Valid referral from your GP or other Specialist.

• All relevant test results including blood tests, urine tests and imaging (e.g. MRI/CT/Ultrasound)

To provide the best care, it is important that Dr Reeves can review all images in addition to reading the radiologist report. Please ensure that you check with the practice team that we can access your images online. Otherwise, you may need to bring a copy of your scans on CD.

• Medicare Card, Concession Card, DVA Card, Private Health Insurance details.
